This Gateway links to a SECURE Credit Card Payment Authorization Form where you enter your credit card and address information.
- In addition to your credit card number, you should enter your three digit "card code" (the last three digits of a printed number on the back of your card, if available). The merchant card processor now requires this for newer Discover cards.
- "Billing address" is the address to where your credit card statements are sent, and is required for security verification by the merchant card processor.
- "Shipping address" is the address to where your puzzle is to be shipped. Not required if the same as your card's billing address.

Completing the Credit Card Payment Authorization Form authorizes payment from your credit card in the amount you enter below, your card is NOT BILLED at this time.

• Prior to using the Gateway, below, you should have been in contact with John and have agreed upon a payment amount and obtained a password. This section is not an initial order form. For international orders, import duties and taxes are not included in the agreed amount and will be your responsibility if imposed by your government.

If ordering a Custom or Gallery puzzle, your credit card will not be billed for the amount you authorize until John ships your puzzle. There may be an exception for international orders.

If you are paying for a 100 Puzzles Project puzzle (eBay), your credit card will be billed and your puzzle shipped in a timely manner.

You will receive an immediate email after you have completed the Credit Card Payment Authorization Form (provided you enter your email address) from Custom Puzzle Craft's merchant card processor. John of Custom Puzzle Craft will send you a separate email, in a timely manner, acknowledging your Authorization.

John will mail you an invoice when your puzzle is shipped. John's invoice will break down the costs of your puzzle, including shipping, taxes and other items per your puzzle order.

Thank you,
J
